PCC will provide 90-day technical support on the digital device issued. If the device is damaged, stolen or no longer operates due to user damage, PCC is not responsible for replacing the device or the SIM card. Each device includes one year of service and assistance with affordable internet options once the one year of service is complete. I acknowledge that I have read this consent form in its entirety, or it has been read (or translated) to me, and I have had the opportunity to ask questions about it and understand it.
I will not rely on any health information I find online as medical advice. I will always seek the guidance of a healthcare professional whenever I have questions about a medical condition. I will contact emergency medical services or go to the nearest emergency department if I have a medical emergency.
